1.Main Job Purpose Ensure safe transportation and the safety of staff and outreach teams. Human Rights Ambassador: Adolescent and Young People Programme – Western Cape

1.Main Job Purpose Improve the legal literacy of young women by educating young women on their rights to confidentiality, informed consent, protection against discrimination, violence and sexual exploitation. Improve the referral of clients to social grant support, medico-legal post violence care and GBV services. Support and coordinate of human tights advocacy  and campaigns, alongside civil society.  To promote healthy  behaviours and prevent risk behaviours in adolescents through; the implementation of targeted interventions at individual, facility and community level in order to enhance uptake of HIV-testing and linkage to and retention to treatment and uptake of prevention services such as PrEP and sexual reproductive health and services. 2. 1.Main Purpose of the Job To deliver high-quality, nurse-led, comprehensive HIV and primary healthcare services to men who have sex with men (MSM) and other key populations through facility-based and community mobile services at Anova’s Ivan Toms Centre, its mobile units, and other DOH sites as needed. The NIMART Professional Nurse will provide integrated clinical care across the HIV prevention and treatment cascade, ensure retention and viral suppression within allocated cohorts, uphold national clinical guidelines and quality standards, and contribute to client-centred, stigma-free services that strengthen health outcomes and continuity of care. Roles and Responsibilities Key Performance Areas Tasks and Activities Clinical Service Delivery, Cohort Management, and Retention Support  Provide standard clinical services at the Ivan Toms Centre, its mobile units, or additional clinic sites as required, including HTS, ART, PrEP, PEP, STI and TB screening, and STI treatment. Main purpose of the role To provide leadership and oversight on the implementation of the NACOSA-funded HIV adolescent and youth programme referred to as the “My Journey” programme aimed at prevention of new HIV infections in Adolescents and Young Women trough targeted activities and awareness campaigns. The “My Journey” programme is a targeted initiative focused on addressing the challenges faced by young women in relation to HIV and AIDS, sexual and reproductive health and wellbeing. The programme offers a defined package of services to adolescent girls and young women between 15 and 24 years old. Young women access these services across a range of community settings, according to their needs, The programme will focus on the following: prioritising HIV testing, consistent condom use, prevention choices and continuity of care. Other activities to increase uptake and access to services include community mobilisation, digital demand creation and youth-friendly information on health and rights and connecting young women to support services such as post-violence care. 2. Duties and responsibilities Program Implementation Develop the My Journey programme implementation strategy and oversee stakeholder engagement, programme inception/start-up, implementation and performance monitoring. Ensure programme deliverables are met against set indicators, and institute correction measures where needed. In doing so, the CLO will provide direct support to the project manager to ensure retention, referral, and linkage activities for ART, PrEP, and general health care are in place for clients and fully implemented per protocol. 2. Conduct case management for all newly diagnosed clients (or clients requested by line manager/clinical staff) according to case management protocols provided by senior clinical advisor to facilitate ART initiation and viral load suppression. Provide comprehensive HIV prevention and treatment education, STI awareness, and PrEP education. Maintenance of Records and Reporting Maintain all communication records and provide weekly/daily reporting on tracking progress, successes, and failures Complete all required DOH, Ivan Toms, and Anova data collection and reporting tools. Maintain detailed daily records of case management progress for each client Complete other administrative tasks or other requirements as requested by the retention coordinator and/or project manager.   3. Minimum Qualifications, Experience and Skills Required Grade 12 or National Senior Certificate Relevant subject specific certifications including HTS counselling. Minimum of 2 years’ experience in the provision of health/support services for key populations required (MSM, preferred) Proven high quality customer service and client engagement experience At least 2 years’ experience implementing PEPFAR-funded activities Demonstrated experience with HCT, ART, and behaviour change counselling required Computer literate & have basic social media skills Strong interpersonal, written, and oral communication skills are required including mobile- and social-based communication apps.
