Community Coordinator: Adolescent and Young People Programme

Main Purpose of the Role To ensure operational, day to day smooth running of community activities. The Programme Manager will have oversight of work done within the community, will track performance and targets within community and will need to do routine reporting. Roles and Responsibilities Outreach supervision Provide a comprehensive package of services through a network of Health and Wellness Counsellors to AGYW, their partners and their other stakeholders through micro planning. Coordinate the activities of interventionists to cover all areas of designated sites under responsibility of the community coordinator. Conduct regular service mapping activities with the interventionists in the community. Ensure all places in community safe spaces have regular access to the My Journey Programme defined package of services, as required. Conduct regular meetings with team of interventionists (e.g. Health and Wellness Counsellors, Peer Educators) to discuss progress and address challenges to continuously improve programme implementation. Monitor the activities of Interventionists and provide necessary support to ensure the performance, confidentiality and commitment. Provide supervision to peer educators and other staff implementing the community component. Address any challenges and performance related issues with the interventionists to the programme manager. Work closely with the peer educators and Health and Wellness Counsellors and Linkage Officers to ensure successful referral and follow-up. Support Interventionists to find AGYW and mobilise for dignity packs and services, as well as identify AGYW eligible for hygiene packs. Conduct regular monitoring and support site-visits to Interventionists Networking, leadership and Advocacy Working with the Paralegals, Human Rights Reactors, liaise with public health facilities, DSD and SAPS – linking, sensitisation and engagement. Work with the Interventionists to mobilise AGYW to understand the importance of being healthy, knowing their HIV status and accessing services and programmes. Represent the programme in identified meetings, as required. Work and support the district Young People’s network, including Y-Key Pops. Programme Coordination Working with the programme manager, ensure the implementation of the operational plan for selected site. Monitor stock meant for AGYW (E.g. hygiene packs and dignity packs) stock levels at community spaces and arrange proactive distribution by the interventionists. Identify gaps of services for AGYW and strategies with the programme manager in an attempt to ensure they are filled. Conduct regular size estimations, complete all micro planning tools and participate in research when requested. Respond to enquiries from internal and external clients about programme activities. Reporting, Administration and Facilitation Verify and collect weekly and monthly reports from peer educators. Working with the data capture, consolidate data and other information from peer educators, prepare and submit monthly and quarterly reports to M&E officer and programme manager. Perform related office administration duties of the programme. Personal development Participate in learning and development opportunities (trainings, workshops, information sessions etc.). Stay abreast of developments in the fields of HIV/AIDS, STIs and TB, including Youth Developments, and Key Populations and Human Rights Organisational responsibilities Attend meetings, briefings, debriefing, staff meetings and other as directed to do so on a regular basis. Client Liaison Officer: 7 Months FTC

1. Main Job Purpose The Client Liaison Officer is responsible for supporting clients in care at the Ivan Toms Centre, its mobile units, and its Call4Care line. In doing so, the CLO will provide direct support to the project manager to ensure retention, referral, and linkage activities for ART, PrEP, and general health care are in place for clients and fully implemented per protocol. 2. Client Engagement and Linkage Serve as one of the primary contacts for clients of the Ivan Toms Centre, its community-based mobile clinics, and/or its Call4care line. Ensure clients are greeted professionally when they make contact/arrive and facilitate their linkage to the appropriate service provider per protocol. Support Ivan Toms Call4Care line during business hours according to established protocols to actively facilitate client linkage to requested services. Provide Testing and Education Conduct HIV counseling and testing and/or HIV Self-Screening distribution in line with Department of Health and Ivan Tom Centre guidelines as required. Provide clients with detailed counseling, including couples counseling, index testing, and risk assessments as defined by client needs and clinic protocols. Conduct case management for all newly diagnosed clients (or clients requested by line manager/clinical staff) according to case management protocols provided by senior clinical advisor to facilitate ART initiation and viral load suppression. Provide comprehensive HIV prevention and treatment education, STI awareness, and PrEP education. Maintenance of Records and Reporting Maintain all communication records and provide weekly/daily reporting on tracking progress, successes, and failures Complete all required DOH, Ivan Toms, and Anova data collection and reporting tools. Maintain detailed daily records of case management progress for each client Complete other administrative tasks or other requirements as requested by the retention coordinator and/or project manager.   3. Minimum Qualifications, Experience and Skills Required Grade 12 or National Senior Certificate Relevant subject specific certifications including HTS counselling. Clinical Manager: Adolescent and Young People Programme

1.Main Job Purpose The role ensures delivery of integrated, high-quality health interventions guided by regular needs assessments, including HIV testing and self-screening, PrEP provision, ARV treatment and viral load monitoring, TB and STI screening and treatment, sexual and reproductive health services, prevention of vertical transmission, post-violence care, and HPV screening. The position is responsible for establishing linkages to care, managing healthcare needs, ensuring treatment adherence and completion, and driving continuous improvement in service delivery in alignment with NACOSA, NDoH, and Global Fund standards. 2. 1. Main purpose of the role The M&E Manager is responsible for high-quality data collection, collation, data quality assurance, data analytics and  reporting data to funders. The M&E manager will  build capacity and will provide leadership to programme implementation team, data management and reporting teams. 2. Main purpose of the role To provide leadership and oversight on the implementation of the NACOSA-funded HIV adolescent and youth programme referred to as the “My Journey” programme aimed at prevention of new HIV infections in Adolescents and Young Women trough targeted activities and awareness campaigns. The “My Journey” programme is a targeted initiative focused on addressing the challenges faced by young women in relation to HIV and AIDS, sexual and reproductive health and wellbeing. The programme offers a defined package of services to adolescent girls and young women between 15 and 24 years old. Young women access these services across a range of community settings, according to their needs, The programme will focus on the following: prioritising HIV testing, consistent condom use, prevention choices and continuity of care. Other activities to increase uptake and access to services include community mobilisation, digital demand creation and youth-friendly information on health and rights and connecting young women to support services such as post-violence care. 2. Duties and responsibilities Program Implementation Develop the My Journey programme implementation strategy and oversee stakeholder engagement, programme inception/start-up, implementation and performance monitoring. Ensure programme deliverables are met against set indicators, and institute correction measures where needed. Provide regular and ad-hoc report on implementation of agreed workplan in line with objectives, indicators and targets. Establish a comprehensive service package including SOPs and algorithms to ensure high quality of service delivery with regards to community AGYW SRHR and PrEP service. Enable availability of resources required for programme implementation, in line with the available budget. 1.Main Job Purpose Addressing the unique challenges faced by young women in relation to HIV and AIDS, sexual and reproductive health and overall wellbeing. Offer a package of health interventions as defined by a needs assessment. The package should include: Regular need assessments to identify services gaps: HIV testing services , including HIV -self screening, sexual and reproductive health education and information , HIV prevention services including providing Prep choices, ARV treatment, case management and viral load monitoring, Identify young pregnant women living wit HIV and prevent vertical transmission, TB and STI screening, diagnosis and treatment, sexual and reproductive health services, Basic package of integrated post-violence care , HPV screening and referral To provide quality health care to ADOLESCENT AND YOUNG PEOPLE PROGRAMME, establish linkage into care channels with different facilities, manage their healthcare needs, and ensure treatment completion. 2. Key Performance Areas ART Initiation, TB, HEP B & C, and other NCDs linkage to care Ensure that all the diagnosed HIV positive clients are initiated on ART daily Build relationships and establish linkage to care channels with different facilities in different regions. Assess the HIV prevention care and support needs of each client and prepare a plan together with the Site Coordinator, Linkage Nurse, HTS Counsellors, and Linkage officers to address these needs. Provide outreach clinical services i.e., HTS, STI and TB screening and Sputums/Xray done, STI treatment, wound care, antiretroviral treatment (ART), PEP, PrEP and adherence support groups done, non-communicable diseases screening, sexual reproductive health (cervical cancer screening and provision of family planning to all female clients), and referral for specialist services offered only at the clinic. Ensure that all clients testing HIV positive and screen TB positive, you collect the sputum for analysis and treatment is initiated until completion Ensure that all medical services provided during outreach need to adhere to National Department of Health guidelines and policies [13], and ensure that we reach and surpass the targets.
